Job Description:
Summary/ObjectivePrimary job responsibilities involve receiving
and coordinating all referrals coming into the CME, managing
reporting, and recording transitions of levels of care within the
CME and exits from the CME. The Program Coordinators will conduct
initial outreach to explain the service, obtain consent for
participation, and link the individual or family/ caregiver to the
appropriate care coordinator. Additionally, the Program
Coordinators will serve as a liaison and support for individuals
and families/ caregivers requesting enrollment in the 1915(c) to
access available services, including submission of prior
authorizations and assisting with the processes related to
accessing flexible funds.Essential Functions Reasonable
accommodations may be made to enable individuals with disabilities
to perform the essential functions. Receive all referrals for
intensive or moderate care coordination and conduct virtual,
in-person, or telephonic outreach to the youth's family according
to the established timeline requirements to explain the service and
obtain consent.Upon receipt of consent, complete all routing of
referrals and cases while maintaining responsibility for the case
until initial assessment with the care coordinator have been
completed.Track efforts to engage youth or family/caregiver that
are ineffective and notify the OhioRISE plan, as indicated, to
re-evaluate the Tier assignment if the individual or family/
caregiver is unable to be reached or indicates they are not
interested in intermediate or moderate care coordination.Monitor,
track, and complete any necessary enrollments, transfers,
disenrollment processing in intensive or moderate care coordination
according the OhioRISE plan processes.For individuals or family/
caregivers requesting enrollment in the OhioRISE 1915(c)
waiver:Complete initial waiver Level of Care assessments, including
the CANS, for individuals requesting enrollment in the OhioRISE
1915(c) waiver.Submit prior authorization requests when the CME
believes a child or youth enrolled in the waiver needs access to
waiver services beyond established service limits.Assist children
or youth and their authorized representatives with connecting with
the waiver’s established Financial Management Services entity
(either the OhioRISE plan or its designee) to exercise budget
authority as needed when the child or youth has a documented need
for Secondary Flex Funds as offered under the 1915(c)-waiver
program.Assure children or youth receiving Tier 2 or Tier 3 care
coordination who are disenrolling from the OhioRISE 1915(c) are
carefully transitioned out of the program by identifying needed
supports for ninety calendar days following disenrollment from the
OhioRISE programComplete annual Level of Care redetermination
assessments in a timely manner for individuals enrolled in the
OhioRISE Waiver who have Tier 2 or Tier 3 Care Coordination.Refer
children or youth and their family/caregivers to ODM’s Central
Processing Team to complete further waiver eligibility
determination steps.Meet engagement, documentation, and contract
requirements as established by management. •Provide excellent
customer service and communication coordination between team
members, individuals, families/ caregivers, community members, and
collaborating organizations.All other assigned duties.Skills,

arms.Position Type/Expected Hours of Work This is a full-time,
exempt position, and hours of work and days are Monday through
Friday, 8:00 a.m. to 4:30 p.m. Occasional evening and weekend work
may be required as job duties demand. TravelTravel is primarily
local during the business day, although some out-of-the-area and
overnight travel may be expected occasionally. Position may require
transporting clients as appropriate to support their linkage
needs.Education/ExperienceBachelor’s degree in psychology, social
work, or other related field OR at least three years’ relevant
experience with the ability to complete additional training, as
necessary.Three years of experience in children’s behavioral
health, child welfare, developmental disabilities, juvenile justice
or a related public sector human services or behavioral healthcare
field, providing community-based services to children and youth,
and their family or caregivers.High School and GED.Experience in
one or more of the following areas of expertise: Family
systemsCommunity systems and resourcesCase managementChild and
family counseling or therapyChild protectionChild
developmentAdditional Eligibility QualificationsValid Ohio Driver's
License with Ohio Bureau of Motor Vehicles - No more than 5 points
on driving record. Valid car insuranceCurrent CANS Assessor
training or the ability to complete such within 90 days of
hire.Training in cultural competency or the ability to complete
