Role:
- This is a DRIVING position serving Columbiana County
- This is a full-time position
- Schedule will include Monday-Friday with a weekend rotation.
Hours are 8:00am-4:30pm.

- And much, much, more! The Personal Care Specialist Essential
Duties Are:
- Reviews Plan of Care prior to making each visit to ensure the
correct care is given to every patient assigned.
- Provides or assists with personal care, including but not
limited to bathing, dressing, oral hygiene, hair care, meal
preparation, feeding, transfers and exercises based on plan of
care.
- Maintains comfortable clean environment by adjusting lighting,
room temperature, changing bed linens and light housekeeping as
needed. Alert to safety factors and accident prevention.
- Reports results timely and accurately to the RN for any vital
signs, or other equipment readings as ordered by the plan of
care.
- Participates as a member of the interdisciplinary team and
communicates with the RN any patient changes or updates to ensure
proper response time and quality patient care.
- Documents activities carried out into the electronic health
record at the point of care.
- Follows individualized plan of care, documenting comprehensive
observations, interventions and all communication with supervising
RN.
- Recognizes and reports changes in mental or physical status of
patient promptly to RN to provide sufficient communication to team
members.
- Assists in other activities and departments when requested to
ensure complete and thorough patient care for all Ohio's Hospice
patients.
- Participates in Continuous Quality Improvement activities to
support the quality of Ohio's Hospice services.
- Precepts new staff to support professional relationships with
newly hired team members as required. Personal Care Specialist
Qualifications:
- High School Diploma or GED
- Certified State Tested Nursing Assistant with State of Ohio
without any board actions
- One year of experience preferred
- CHPNA certification required or after completion of required
hours for certification
- Computer skills sufficient to properly document services and
care
- Ability to drive during daytime, nighttime or inclement
weather.
- Valid Driver's License with Safe Driving Record
- State Minimum Automobile Insurance Coverage
