Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: ASAP - Duration: 13 weeks - 40 hours per week - Shift: 8 hours - Employment Type: Travel Title: Physical Therapist (PT) – Home Health / Hospice Location: Dublin, OH Duration: 13 weeks Shift: Day shift, 5x8 hours (8:00 AM – 5:00 PM) Job Responsibilities /t - /t Provide physical therapy services to geriatric patients in a home health/hospice setting /t /t - /t Manage an average caseload of 5–7 patients per day /t /t - /t Complete Start of Care (SOC) visits when required /t /t - /t Perform timely, accurate clinical documentation and maintain clear communication /t /t - /t Collaborate with interdisciplinary care teams /t /t - /t Participate in on-call rotation (approximately every 6th weekend) /t /t - /t Travel within an assigned territory up to a 60-mile radius /t /t - /t Adhere to professional standards, policies, and dress code requirements /t Required Skills, Certifications & Qualifications /t - /t Minimum 1 year of recent Physical Therapy experience /t /t - /t Home Health and/or Hospice experience required /t /t - /t OASIS experience required for SOC visits (Home Health) /t /t - /t Proficiency in HCHB (Homecare Homebase) EMR required /t /t - /t BLS certification required /t /t - /t Active PT license in hand at time of submission /t /t - /t Valid driver's license and reliable personal transportation /t /t - /t Strong time management, documentation, and communication skills /t /t - /t Ability to work independently in the field /t /t - /t Willingness to be flexible with territory coverage /t /t - /t NO first-time travelers /t HonorVet Technologies Job ID 31693852.
