Job Description:

We are seeking a Nurse Practitioner (PRN) for home risk assessments, welcoming new grads.
Conduct assessments (up to 1 hour) reviewing health, medical history, medications, and social factors to improve care quality and identify undiagnosed conditions.
Collaborate with physicians, case managers, and caregivers in various settings including home visits and skilled facilities.
Requirements include a Master--------s or equivalent experience, current RN and NP licensure with prescriptive authority, board certification, and at least 1 year NP experience.
Flexible schedule, travel required to patient homes, with compensation from $2,300 to $11,500/month based on completed assessments. Benefits include mileage reimbursement and 401(k).
