Job Description:
OVERALL OBJECTIVE OF JOBThe primary function of this position is
to ensure the total delivery of nursing care and related services
for all residents under their assigned area and in compliance with
all federal, state and local guidelines, standards and regulations
which govern the facility, and as directed by the Nursing
Supervisor and/or Director of Nursing Services so as to ensure each
resident's medical, psychosocial and spiritual needs and wishes are
met effectively and efficiently on a continuum.ESSENTIAL FUNCTIONS
OF JOBGENERALl. Plans, provides and directs resident care according
to physician orders and the Interdisciplinary Plan of Care.2.
Completes resident assessments and develops/updates the
Interdisciplinary Plan of Care in conjunction with the RN
Supervisor and RNAC.3. Transcribes MD orders as needed.4. Makes
daily resident rounds and provides verbal and written reports to
the RN Charge Nurse.5, Maintains the Kardex to ensure up to date,
current information.6. Assists the RN., in completion of the
reporting process following a resident incident.7. Maintains a safe
work environment and exhibits safe work practices.8. Assist
subordinate positions in rendering of care and services as
necessary and/or directed.9. Answers resident call bells,
regardless of assignment.10. Adheres to, up-holds and enforces all
facility policies and procedures.11. Completes various
documentation and reports as directed.Administrative12. Prepares
the CNA and LPN daily assignments for the assigned shift.13.
Monitors and supervises the work of the Nursing Assistants so as to
ensure the needs of each resident is met on a continuum.14.
Provides written and/or oral disciplines and education to nursing
assistants.15. Assists in scheduling, training, orientation of new
employees and recommends disciplinary action as warranted.16.
Monitors and supervises the unit ward clerk.17. Attends all
department meetings and/or educational programs as scheduled and/or
directed.18. Actively participates in overall facility management
and makes recommendations for quality improvement.19. Reports any
supply/equipment needs, repairs, etc., to the appropriate
department.20. Reviews policies and procedures, makes
recommendations as warranted.21. Adheres up, upholds and enforces
all regulations and recommendations of the Department of Health,
the Health Care Financing Administration, the Department of Public
Welfare, etc.22. Participates on various committees and with
in-service training as directed.SUPERVISION RECEIVEDReceives
occasional instruction and some supervision from Nursing Supervisor
in regard to daily work duties.SUPERVISION GIVENProvides
supervision to nurses aides, ward clerk and volunteers as delegated
by Nursing Supervisor.WORKING CONDITIONS1. Works indoors in
adequate work space and lighting, but with fluctuations in
temperature and ventilation.2. Works indoors with average exposure
to noise, but subject to frequent disruptions and stress3. Normal
indoor exposure to dust/dirt.4. Periodically works beyond normal
work hours or works on-call or on an as needed basis.5. Potential
exposure to bloodborne pathogens or communicable diseases.6. Works
in conditions of potential outbursts of disruptive behavior of
clients.ESSENTIAL PHYSICAL/MENTAL/STRESS DEMANDS1. Frequent
movement about the work site during the workday.2. Frequent lifting
of 20-30 pounds.3. Climbing/Balancing required.4. Stooping/Bending
required during normal workday activities.5. Standing,/Sitting
required during normal workday activities.6. Reaching above level
of head required during normal workday activities.7. Normal hearing
ability required.8. Speaking/Communications with normal ability
required.9. Vision with correction to 20/20 required10. Position is
stressful.QUALIFICATIONSEDUCATION/TRAININGCurrent LPN license.
(Graduate from approved school of practical nursing) B.WORK
EXPERIENCEThree to five years of LPN nursing experience, preferably
in long term care. At least one year experience as a medication
nurse and treatment nurse.KNOWLEDGE SKILLS AND ABILITIES REQUIREDl.
Must be able to speak and understand the English language and to
follow oral and written instructions.2. Must possess effective oral
and written communications skills.3. Must possess initiative and
problem solving skills.4. Must possess ability to function
independently, have flexibility and personal integrity and the
ability to work effectively with clients, co-workers and others.5.
Must possess ability to maintain confidentiality in regard to
client information and records.6. Must possess knowledge of and
ability to safely perform nursing procedures as allowed by State
Practice Act.7. Must possess knowledge and skill in use of medical
equipment and safe administration of medications.8. Must possess
ability to respond to emergency situations, to practice CPR and
First Aid as necessary.9. Must possess knowledge of practical
nursing theory, principles and practices, and some knowledge of
patient care methods and techniques of the geriatric population.10.
Must possess ability to practice application of practical nursing
methods and techniques and to follow established policies and
procedures.11. Must possess ability to maintain nursing and patient
care records and reports.12. Must possess ability to interact
effectively with supervisors, co-workers, patients and their
