Altimate Care is hiring! Are you a RN that is tired of running the halls, being in understaffed facilities, and overwhelmed, stressful environments?
Altimate Care provides home health services to our patients by delivering one on one nursing care in our patients' homes. This helps promote healing, reduces stress and allows the client to remain in their home. We're seeking a RN Case Manager to join our team!
Responsibilities
- Initiates, implements, makes necessary revisions, and provides services in accordance with the plan of care
- Educate patients, families, and caregivers on disease management, home safety, medications, and dietary needs to avoid hospital stays

Communicates changes in the patient's condition and/or needs timely and accurately to care team
Participates in weekly care conferences
Participates in peer review, quality assessment and the performance improvement process

- Monitors other team members per Medicare guidelines

Completes appropriate visit records timely and accurately

Qualifications
- 1-year experience in direct patient care/home health care
- Must be an Ohio Registered Nurse (BSN or ADN) with valid license
- Demonstrate efficient and effective oral and written communication skills
- Ability to relate positively and favorably with patients and staff

Must have a working knowledge of CMS guidelines
Must be reliable, kind and compassionate
